Dully filled-in application form accompanied by paid fee challan, copy of CNIC, 3 recent photographs, attested copies of experiences certificates, educational credential, certificates, diplomas, degrees from recognized institutes and Disability Assessment Certificate issued by the Assessment Board of Social Welfare Department be submitted directly to NTS on the address mentioned on application form.
